How do I prepare for my first visit?Prior to your first appointment, you will receive a welcome email that includes a short welcome video, your intake forms, and a Nourishly link code. The intake forms will be sent via DocuSign (look for an email from DocuSign). Please be sure to complete the forms and watch the video before your appointment. You’ll also see a Nourishly link code, feel free to download the "Nourishly- Nutrition & Diet" app and use the link code to connect with your Dietitian and begin logging your meals. This is a Medical Nutrition Therapy appointment, so make sure to join from a quiet, private place where you can take notes and have access to your computer if needed.

What can I expect at my first visit?At your first appointment, you will meet with your Registered Dietitian. This appointment will take approximately a 30 minutes- 1 hour (depending on your insurance coverage). They will conduct a Nutrition Assessment and get to know more about you, so be ready to be asked questions about your medical history and eating behaviors. You will also have a chance to get to know your dietitian, your partner in this journey to better health and wellness.

What is the process for follow-up appointments?Follow-up appointments are highly recommended. There are many important nutrition education topics to cover over time, and follow-ups allow your Dietitian to make personalized adjustments to your nutrition plan based on how your progress has been since your initial visit. They also provide accountability and ongoing support as you work toward your goals.

How often are follow-up appointments?Follow-up appointments are typically scheduled once per month, but this can vary depending on medical necessity, insurance coverage, and your personal preferences- they can be scheduled more frequently or spaced further apart if needed. Your Dietitian will help you determine what follow-up schedule is right for you and will book your next appointment at the end of your initial assessment.

Why was I contacted to make an appointment?We are a referral based service so if you are contacted by us to make an appointment that means you were referred by your physician.
